  • Attending physician's statement

  • Medical records (admission notes, discharge/death summary, indoor case papers, test reports etc. if applicable)

For accidental or unnatural death

  • Attending physician's statement

  • Copy of the First Information Report (FIR) or Panchanama/Police complaint

  • Copy of the postmortem report (Autopsy report) & Viscera report, if performed

  • Copy of the Final Police Investigation report (FPIR) or the charge sheet

     

Hospitalization & dismemberment claim

  • Copies of hospital bills for the confinement & copies of all related clinical reports pertaining to the claim event

    • Attested true copy of Indoor Case Papers of the hospital

    • Discharge summary of present and past hospitalizations

    • Photo identity of life assured with age and address proof (List of acceptable ID and address proofs of claimant)

    • Bank details of the claimant – Cancelled cheque (with printed name and account number) / bank passbook and NEFT form

    • Medical examination certificate (First consultation notes)

    • All related clinical reports pertaining to the claim event

      • Laboratory test reports

      • X-Ray / CT Scan / MRI reports & plates

      • Ultrasonography report

      • Histopathology report

      • Clinical / hospital reports

      • Angiography reports & plates

      • Certificate of Diagnosis

    • All follow-up consultation notes in relation to the hospitalized condition

  • For claim submission, there are only three requirements:

    1. Valid records/proof showing death of the insured member due floods, cyclones, or other natural calamities
    2. Claimant’s Statement (to be filled by nominee) with bank details of the nominee.
    3. Photo ID, Address proof of nominee and Relationship proof of the nominee with the insured

What if the Nominee if out of India? How will the claim be processed?

The Nominee has the option to submit the claim online by uploading the signed / self attested copies or submit the documents via email.

If the original Policy Contract was lost and request for issuance of duplicate was not made, will the claim be processed without the Policy Contract?

Yes, claim would be accepted as Original Policy Contract is not mandatory for submission of Claim. Declaration is to be provided by the claimant on a plain paper regarding the misplacement of the original policy document. 

How much time will my claim take?

It is our endeavour to settle your claim in an expedited manner. We also offer guarantees subject to the criteria being met

 

  • Death Claim (Without Investigation)  - Regulatory TAT 30 days from the date of receipt of all required documents.  
  • Death Claim (With Investigation) - Regulatory TAT 90 days provided to complete investigation there after 30 days to decide & process claim payout

  • 4 Hours express claim settlement is applicable to only non-early claims with more than 3 years of policy duration, non-investigation cases, up to Sum Assured of Rs. 50 lakhs. Applicable for branch walk in. Time limit to submit claim to Tata AIA Life Insurance is 2 pm on working days. Subject to submission of complete documents. Not applicable for ULIP policies and open title claims.
